Whenever we can we are always pleased to assist our customers. This document gives details of our terms and conditions. If, however, you've any queries or need clarification, please contact a part of staff and us will be pleased to help you.

Please take note that any information supplied through any part of our web site (examples: forum, blog, news, chief site, live chat, phone, e-mail, ) is for fun & amusement purposes only and does not constitute professional advice. No professional advice can be given with no clinical consultation with a dentist.

Dream Smile Dental Clinic does not have a contract with a NHS Primary Care Trust and all treatments are supplied on a private basis only.

Treatment planning:

We shall supply printed details of your plan once your treatment plan has been agreed with the Dentist. Whether this plan shifts on account of radiographic or clinical findings, we shall notify you and discuss this with you. Treatment plans are not invalid for net 90 days from your date the treatment was prescribed. Please do not hesitate to ask us, before the appointments for actual treatments are made, if there are any points in your treatment plan which you want to query.

Approval forms:

Specific treatments require completion of a written consent form. This is before these treatments are completed in order to describe the treatment, aftercare and any threat to you personally thoroughly.

Fees:

A credit account system will not run and we require fees to be settled at the appointment.
Where treatment incurs a laboratory fee, at least 50% of the total fee is due at the appointment where impressions are taken. Fees for specific treatments like Invisalign and Dental Implants are taken at each visit in payments that were staged.

Where treatment is paid for on finance through a Finance company, please take note the finance arrangement must be accepted and signed just before the beginning of that treatment. Take note a minimum deposit may also be required, and that finance is only available on treatments finished?400, not including Emergency Dental Treatment.

If you've financed some of the costs of your treatment through the finance company, and want to cancel your treatment after signing the deal forms, please be aware that a cancellation fee of up to 15% of the entire finance sum will be charged.

Pearl Dental Clinic does not accept any payments.

Fees for treatment where intravenous sedation is included, must be settled prior to signatures being required, or the appointment in order to avoid fiscal trades, whilst a customer is still under the influence of the sedative.

Deposits are not nondeductible off the price of treatment.

All the fees employed for any form of Dental treatment at the practice are based on; the substances used, laboratory costs incurred (if applicable), and the clinical time spent. The fees are yet, illustrated as treatments prescribed on the treatment plan for your information.

Late cancellation or appointments that are missed:
Dream Smile Dental Clinic reserves the right to bill up to?100 per hour, in the event of an appointment canceled with less than 24 hours notice or a missed appointment.

We recognize that some patients travel long distances to get to the practice, and in some events being late for appointments can be ineluctable. If you are more than 10 minutes, please take note that you may be requested to reschedule your appointment.

Guarantee:

Ceramic Veneers, at Dream Smile Dental Clinic our Lumineers, Implant crowns and Ceramic crowns are ensured for an interval of 5 years following placement, provided that the following conditions are satisfied:

The treatment has been completely paid for by the patient and does not owe the practice any money for the treatment received.
The Veneers, Lumineers, Crowns & Implant crowns haven't been damaged as an effect of an accident, excessive grinding or injury.
The patient has followed all post treatment maintenance recommendations.
The patient has attended and regular evaluation every 6 months.
Some treatments may have a guarantee of less than 1 year, and in this case you'll be advised by your Dentist either verbally or in writing, or both.

Personal Details:

It's crucial that you simply give the full medical history and details of any drugs you take. Should these change in any way, it is extremely important that you tell your Dentist. It's the patient's obligation to inform the practice of any changes in private details and/or their medical history.

Use of Pictures and X-rays:

Dream Smile Dental Clinic may use images and x-rays of your smile and teeth just, for educational and promotion goals on the Dream Smile Dental Clinic site, and on promotional and educational literature. Your name WOn't ever be printed, and identity will never be revealed. However, should you NOT want for us to use your images and x-rays in this manner, please let's know.

Use of patient contact details:

We like to remind other various significant reminders, and our patients of their appointments, when they are due for appointments. On this note, you might be occasionally contacted by the practice via phone, text, e-mail or by letter in the post. If you DO NOT want to be contacted by the practice all or by any of these means, please inform US.

Grievance's policy:

To be able to ensure that every patient has just the very best experience constantly, at Dream Smile Dental Clinic we constantly take complaints about any aspects of our services very seriously.

Criticisms can be made in writing by filling out a simple complaints form available or via e-mail through our on-line site contact form, by the patient or by an authorized person on the patient's benefit. Criticisms should be made to the Dream Smile Dental Clinic Management', and should be clear, so that they can be dealt with efficiently.

Every complaint will receive immediate written/e-mail acknowledgment, and the Management will strive to resolve the complaint within a fast, reasonable period of time (normally about 2 weeks). For our complete complaints procedure and policy, please ask a member of our reception team. It will usually be admitted by the Management team and after that referred to the treating Dentist if a complaint is about any aspect of clinical care or charges that are affiliated. Take note that in these scenarios, no liability is accepted by Dream Smile Dental Clinic on behalf of the treating Dentist and acts as a liaison between the patient and the treating Dentist, only.

If you are unsatisfied with our internal management of your complaint, you are free to forward your complaint to the Dental Complaints Service (www.dentalcomplaints.org.uk). If still unsatisfied, you may want to forward your complaint to the General Dental Council.

No tolerance/Mistreatment policy

In these scenarios, the right to deny treatment and entry is reserved by Dream Smile Dental Clinic.

Data Protection Act:

We store all patient personal details on a protected computer system relative to the Data Protection Act. All clinical notes, digital radio-graphs digital photos etc, remain the property of Dream Smile Dental Clinic. Copies of notes, radiographs and photographs can be made available on request, and Dream Smile Dental Clinic reserves the right to charge an administration fee for these.
